We understand that circumstances leading to residential placement are both unique and often times, very personal. When the decision to place is made, we begin the evaluation process in a sensitive and confidential manner. All information, written and oral communication, is handled by the Director of Admissions and in most cases, we will conduct the pre-placement evaluation at or near an individual’s current location.
Upon completion of a pre-placement interview, the Admissions Department will make a final determination and the referring agency will be informed of a decision. All referrals for service are considered without regard to race, color, religious creed, disability, handicap, ancestry, national origin or age.
With each request for placement, the Admissions Department requires some pre-placement information which includes the following:
- A psycho-social history to include the family structure and dynamics; presenting problems; treatment efforts to resolve the problems; and specific services and supports that are needed.
- A current psychological evaluation.
- A current psychiatric evaluation as needed.
- Individualized Education Plan.
- Medical history and insurance information.
- A completed application form. (see below for details)
- A complete physical exam.
