The Keystone Community Resources Transitional Program assists young adults with the development of real life transitional skills as they move from late adolescence to adulthood. We specialize in services for individuals diagnosed with developmental disabilities, emotional, mental health, and behavioral problems.
Young adults that have been identified to have disabilities often lack exposure to real life living experiences. Too often, these individuals fail because they do not have the skills that will allow them to survive in today’s society. During this transitional period lasting from early adolescence to young adulthood, these individuals need to receive transitional training that is more associated with “real world” experience. Our program will offer youth just that; hands on training, and exposure to daily living skills. This training will not only meet their transitional needs, but will give them a strong ethical foundation to begin the construction of their future. Through these experiences, we intend to better prepare participants to deal with the everyday challenges that the transition to independent living and adulthood will bring.
This program provides 24-hour supervision in a community-based group home setting located in North East Pennsylvania. We utilize masters level, bachelors level, and paraprofessional staff to help the individuals we serve address their developmental needs, day to day problems, and learning disabilities in a caring and compassionate setting.
The goal of this program is to provide these young adults with the skills to reach their optimum level of functioning in the community. For some individuals, this goal may consist of movement into independent living setting, while others may only have the necessary capabilities for placement in a more long-term style adult group home.
In order to meet each individual’s goals, the Transitional Program provides educational services through the local Intermediate Unit {NEIU #19), vocational training and development, community job placement, exposure to community based resources, life skills training, therapeutic based recreational activities, individual and group psychotherapy as needed, relationship building, psychiatric and psychological services, and off-site health services. In addition, the program may provide alternative services as needed such as specialty assessment and services through one of our various referral sources.
The Keystone Community Resources Transitional Program provides a treatment team approach as a method for assisting each individual in meeting their treatment goals. This structure will encourage support and assistance from the county case manager and family. Upon admission, the individual will take part in an assessment process that will identify goals of treatment. After 30 days, the individual will assist in the development of a Service Plan that will identify techniques for reaching the goals that have identified.
In order to be considered for a move to a more permanent setting, the individual must demonstrate success in the program which is to include notable progress in goal areas, as identified by service plan, as well as successful involvement in a vocational area most appropriate for their level of functioning. At this time, a referral may be made to alternative living arrangements, with the input from the individual. These resources may include more permanent group home placements, supported living programs, or an independent living situation.
